Social Determinants of Health: Turning Compassion into Action

This course is designed to educate attendees on assessing, identifying and providing appropriate interventions for individuals who are experiencing Social Determinates of Health. Social Determinates of health are non-medical factors that influence outcomes of health, such as socioeconomic status, education level, access for quality housing, food insecurity and safety. Data will be presented to show risk factors of those experiencing these challenging situations. Social workers will be educated on the early identification process to facilitate better understanding and implementation of appropriate interventions and referral processes. Community based partnerships and engagement strategies will be reviewed to provide education on appropriate referral processes.

  • Appropriately assess and identify risk factors in those experiencing Social Determinates of Health
  • Learn about community-based resources to assist those with Social Determinates of Health to improve outcomes
