Cause Analysis Training - January 17, 2025
Press Ganey will provide a methodical framework, techniques and tools for RCA that can be applied to enhance cause analysis in any setting or department.
Target Audience
Physicians, Nurses, Quality & Patient Safety Professionals
Learning Objectives
- Discuss how human performance is influenced and managed in complex systems.
- Define Cause Analysis fundamentals, with a deep-dive tutorial on the 20 Individual Failure Mechanisms and 26 System Failure Mechanisms needed to get to true root causes and how to effectively interview individuals and groups to identify the presence of the different failure mechanisms.
- Explain Training and skill development of single-event RCA methods, including Events and Causal Factor Charting
- Discuss Health Equity Contributors and the impact they have on patient experience and outcomes.
