- Define Cause Analysis fundamentals, with a deep-dive tutorial on the 20 Individual Failure Mechanisms and 26 System Failure Mechanisms needed to get to true root causes and how to effectively interview individuals and groups to identify the presence of the different failure mechanisms.
- Explain Training and skill development of single-event RCA methods, including Events and Causal Factor Charting
- Discuss Health Equity Contributors and the impact they have on patient experience and outcomes.
