STATEMENT OF NEED
This PCG webinar series was designed to streamline knowledge of subspecialized diagnoses/programs as well as increase provider connectivity to TMC across the system. The goal is to have the learners engage with the subspecialized program specialists for future patient's continuity of care.

At the conclusion of this activity, the participant should be able to:
- Have more knowledge about the subspecialized programs available at Houston Methodist Hospital (TMC)
- Use best practices around management of patients pre/post-referral from specialist's perspective
- Feel increased connectivity to Houston Methodist Hospital (TMC)
- Engage with specialists directly/1:1 and discuss specific patient cases
