  1. Review what are Social Determinants
  2. Discuss how they impact health care being provided in acute care settings.
  3. How do we document Social Determinants in the medical record?
  4. How can Palliative Care and Support Services address Social Determinants of health care.

Nursing Learning Outcome:

On completion of this activity, registered nurses will be able to describe one concept they will be able to incorporate into their bedside practice related to their learning about social determinants of care as related to the palliative care setting, as evidenced by self-reported intent to change practice as stated on the post-activity evaluation tool.
