At the end of this activity, the learners will:
Discuss the different phases of Transition Shock Theory as outline by Duchscher.
Identify the responses commonly experienced during the transition shock.
Name common stressors and early transition challenges.
Apply evidence-based decision-making models to patient care.
Use SBAR and other structured communication tools when interacting with interdisciplinary teams.
Discuss effective coping mechanisms for managing stress during the transition period.
Identify the core components of hepatology nursing role.
Identify the nine-liver care competencies.
Discuss the pathophysiology of common liver diseases associated with liver cirrhosis.
Use clinical data to identify complications of cirrhosis
Explain the End-Stage Liver Disease Care Pathway from initial evaluation through transplant candidacy, and transplant phase.
